颈椎骨关节病变引起的眩晕的病因：因外伤、劳损或椎间盘退变、椎间隙变窄所致的颈椎失稳，是产生症状的基础。
在失稳的情况下，颈椎小关节易产生错位，颈椎旋转、颈椎超常范围的活动；颈椎管内、外软组织也容易继发性无菌性炎症或形成颈椎体、小关节、钩突关节的骨唇增生。位于颈椎椎体前外侧的植物神经链或走行在颈椎横突孔中的椎动脉，受到无菌性炎症的刺激、机械性压迫或植物神经受到刺激。会引起椎动脉反射性痉挛。此时椎动脉血流受阻，使小脑，前庭神经核、红核等部位的眩晕中枢血液循环发生障碍而产生眩晕症状。
